Dark red box, with on the top of the lid three squares, of which the outer two with gold paint; inside of the lid lined with red silk, with inscription in gold-colored letters; inside of the base lined with burgundy-red velvet, in which a round cutout has been left. Accompanying medal NG-165-1.
